The patient, with a medical history of diabetes mellitus, hypertension, and bladder carcinoma (post-TURP), was admitted with severe neurological symptoms including headache, confusion, drowsiness, and involuntary facial movements.

CT and MRI scans ruled out acute stroke or bleeding, while confirmatory tests revealed NMDA receptor-positive autoimmune encephalitis in both cerebrospinal fluid and serum.

5-Day IVIG Protocol Proves Effective in Recovery

Treatment began with high-dose steroids (1 gm Solumedrol IV for 3 days) and was followed by five cycles of Intravenous Immunoglobulin (IVIG) therapy.

The case required a multidisciplinary approach involving neurology, endocrinology, psychiatry, and critical care departments, ensuring comprehensive management of both the autoimmune condition and the patient’s comorbidities.

“NMDA receptor encephalitis is rare in elderly males and demands early diagnosis. Timely initiation of steroids and IVIG can be life-saving, as seen in this case,” said Dr. Yogesh Godge, Consultant Neurologist at Jupiter Hospital, Thane.
