Mumbai: Demonstrating excellence in advanced obstetric and reproductive care, a multidisciplinary medical team at Wockhardt Hospitals, Mumbai Central successfully managed a high-risk IVF pregnancy in a 48-year-old woman, culminating in the safe delivery of a healthy baby despite multiple maternal and fetal risk factors.

The patient conceived through IVF with ICSI (Intracytoplasmic Sperm Injection) on December 30, 2024, following a history of one prior abortion.

The high-risk IVF pregnancy was medically complex due to severe oligospermia in the male partner and several maternal challenges, including advanced maternal age, fibroid uterus, adenomyosis, thin endometrial lining, low AMH levels, and an ovarian cyst.

High-Risk IVF Pregnancy Managed Through Advanced Surgical and Medical Care

To improve uterine receptivity, the patient underwent hysteroscopic surgery with fundal and lateral metroplasty in November 2024, creating an optimal uterine cavity for implantation. Notably, conception occurred in the very first IVF cycle.

The pregnancy course remained clinically challenging. The patient experienced first- and second-trimester bleeding episodes that required strict bed rest and progesterone support.

She also developed pregnancy-induced hypertension, necessitating continuous monitoring and antihypertensive treatment, along with gestational diabetes that was managed through careful surveillance and multidisciplinary coordination.

Given the elevated risk of thrombosis, she was administered Low Molecular Weight Heparin (LMWH) and underwent regular ophthalmological and systemic evaluations to monitor hypertension-related complications.

Through vigilant follow-up and sustained medical intervention, both maternal and fetal conditions were stabilized.

At 35.4 weeks of gestation, the patient developed premature rupture of membranes (PPROM). After receiving medication to enhance fetal lung maturity, she was taken for a planned Caesarean section.

During surgery, a rare and life-threatening complication – uterine inversion caused by an adherent placenta – occurred. The surgical team responded immediately, manually correcting the inversion and stabilizing the patient.

A healthy male baby was delivered, cried immediately after birth, and did not require NICU support. Both mother and baby were discharged in stable condition five days later.
